About the Job

We are seeking a dedicated Navigator Role in Healthcare to join our team at Primary Care 24. In this role, you will be responsible for providing exceptional care navigation services to patients and supporting the primary care team.

Key Responsibilities

  • Providing high-quality care navigation call handling duties in line with PC24 processes.
  • Communicating effectively with patients and the primary care team.
  • Actioning follow-up calls and providing operational support to the onsite practice team and clinical team.
  • Ensuring patient safety by following established processes and procedures.
  • Recording essential information accurately and concisely.
  • Utilizing effective questioning and active listening skills to achieve optimal patient outcomes.
Requirements
  • A minimum of GCSE Level C in literacy and numeracy or equivalent work-based experience.
  • Excellent communication skills with diverse individuals.
  • Proficiency in Microsoft Office packages and excellent keyboard skills.
  • Patient-focused skills with the ability to demonstrate patience, empathy, and compassion via telephone.
Estimated Salary

The estimated salary for this role is £25,000 - £30,000 per annum, depending on experience.
