Job Summary

The Advanced Primary Care Practitioner will provide high-quality care to patients within a primary and community care setting in accordance with CQC regulation, practice protocols, and policies.

About the Role
  • Provide exceptional patient care and support people living with long-term conditions through training and development opportunities.
  • Develop skills, knowledge, and experience in specialist practice nursing areas as required.

The successful candidate will be able to work autonomously and as part of a wider multi-disciplinary primary care team.

Key Responsibilities
  • Diagnose and manage acute and chronic conditions, ensuring best practice delivery for registered patients while developing General Practice Nursing duties.
  • Support patients in adopting health promotion strategies that promote healthy lifestyles and apply principles of self-care.
  • Promote and deliver evidence-based care for patients presenting with aural conditions.
  • Childhood and Adult immunisations and vaccinations, including Influenza and COVID vaccines.
Requirements
  • Current NMC registration.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
Salary

£35,000 - £45,000 per annum, depending on experience.
