Job Summary

We are seeking a highly motivated and experienced registered professional to join our team as a Mental Health In-Reach Clinical Team Leader. As a key member of our team, you will be responsible for providing leadership and direction to our In-Reach Team, ensuring the delivery of high-quality patient care and services.

Main Duties and Responsibilities
  • Provide effective day-to-day leadership and direction to the In-Reach Team, ensuring the delivery of high-quality patient care and services.
  • Ensure the service is well-organised, evidence-based, and service-user-focused, with a strong emphasis on clinical excellence and patient safety.
  • Support the Service Manager in implementing service changes, ensuring effective implementation with measurable outcomes and agreed timescales.
  • Advance innovative ideas to enable staff to deliver care more effectively, with a focus on improving patient outcomes and experiences.
  • Ensure that clinical practice in the team is delivered safely and in accordance with the Care Quality Commission Framework, Professional Practice standards, and any other relevant national policies and standards.
  • Participate in audit processes as required, ensuring that recommendations resulting from audits are implemented.
  • Provide advice to individual nursing and allied professional staff, as well as other stakeholders, on issues relating to clinical competence, nursing standards, and procedures.
  • Ensure that nursing practice and affiliated professionals' practice is sensitive to the needs of an ethnically diverse population.
  • Ensure that clinical notes are maintained to the agreed standard at all times, and that clients are appropriately assessed, care planned, and discharged.
  • Ensure that data is entered onto systems according to agreed guidelines, and that data collection is prioritised at all times, with incomplete or inaccurate data submission rectified without delay.
  • Manage all complaints and serious incidents, undertaking the relevant investigations as required by Trust Policy.
  • Support the Service Manager in establishing an efficient working environment, making the most effective and efficient use of staff, supplies, equipment, and services.
  • Effectively supervise staff, ensuring safe levels of staffing and management are provided at all times, and advising the Service Manager when this is not the case.
  • Ensure that all staff receive line-management supervision and appraisals on a regular basis, in line with the Trust's supervision and appraisal policy.
  • Ensure that clinical supervision is provided, complying with each of the relevant disciplines' professional regulatory requirements, and thus ensuring that a high standard of professional practice and good inter-disciplinary working is achieved.
  • Ensure that all staff are fully aware of and understand their contribution to achieving successful client outcomes and performance targets for the service.
  • Lead on and participate in the interviewing, selecting, orientating, and developing of new staff within the team, ensuring that they receive a full and effective induction, and that mandatory training and other training needs are met.
  • Facilitate a multidisciplinary approach (including partner agencies if required) and consider different skill mix solutions within the team.
  • Work closely with the Service Manager in the assessment, provision, and evaluation of training needs of nurses within the team.
  • Ensure their own continual professional development, keeping up to date with developments in nursing practice and participating in training programmes identified in an agreed Personal Development Plan.
Requirements
  • Registered Nurse, Allied Health, or Social Care Professional.
  • Education to degree level, or appropriate experience.
  • Substantial post-registration experience.
  • Experience at Band 6 in a multi-disciplinary staff team, including line management and supervision responsibility.
  • Experience of supporting staff through change.
  • Experience of multi-professional collaboration, including partnership working with other agencies.
  • Experienced in involving service users and carers in service planning and monitoring.
  • Competent in using IT, including word processing, spreadsheet, and email.
Desirable
  • Experience within a prison or low, medium, or high security hospital setting.
  • Experience of service development.
Skills and Knowledge
  • Up-to-date knowledge and understanding of current Mental Health Policy issues, including the Mental Health Act, Mental Capacity Act, and Care Act.
  • Up-to-date knowledge of nursing/Allied Health Professional developments, especially in forensic mental health.
  • Ability to provide high-quality clinical advice.
  • Ability to support in building and developing teams of people, and to provide clinical leadership to a diverse workforce.
  • Demonstrable ability to performance manage staff to meet key performance indicators (KPIs) and performance targets.
  • Ability to identify problems and use initiative and creativity in problem-solving.
  • Ability to communicate effectively in writing and orally, presenting ideas with clarity for a variety of audiences.
  • Ability to manage pressure and changing and conflicting priorities effectively.
  • Ability to support and monitor systems for clinical practice.
  • Able to influence and use negotiation skills to manage differences constructively.
  • A demonstrable ability to work closely with a range of stakeholders and other providers in helping plan the future of service provision.
  • Able to demonstrate a genuine commitment to principles of equal opportunity, fair access, and service-user empowerment, and the ability to translate principles into practice.
  • Able to demonstrate an understanding of safeguarding practice for both vulnerable adults.
  • To demonstrate professional integrity, and an ability to present the nursing viewpoint within appropriate meetings.
