We are seeking a dedicated and compassionate Mental Health Support Specialist to join our team at Achieve Together. As a key member of our community care team, you will provide vital support to individuals in their homes and local communities.
**Key Responsibilities**
- Support individuals with mental health needs in their homes and communities
- Work collaboratively with our team to provide a comprehensive support service
- Develop and maintain relationships with service users, their families, and other stakeholders
- Contribute to the development and implementation of individual support plans
