About the Role

We are seeking an experienced social worker to join our team as a Care Coordinator for children and young people with disabilities.

The ideal candidate will have a strong background in social work, excellent communication skills, and a passion for working with vulnerable populations.

This is an exciting opportunity to make a meaningful difference in the lives of our service users, their families, and caregivers. As a key member of our team, you will be responsible for developing holistic care plans, ensuring seamless service delivery, and advocating for the rights of children and young people with disabilities.

Key responsibilities include:

  • Developing and implementing individualized care plans
  • Working closely with colleagues and multi-agency partners to ensure effective service delivery
  • Liaising with external partners to ensure efficient service provision
  • Writing high-quality reports for presentation to senior managers

We offer a competitive salary range of £39,951 - £46,041, plus a generous pension and relocation package, annual leave up to 27 days, and excellent opportunities for career progression.
