Children and Young People Disability Support Worker

**Job Description**
Join our dynamic team as a Social Worker in the Children & Young People with Disability Service. In this role, you will work with children, young people, and their families to develop and implement personalized support plans that promote independence and well-being.
You will be responsible for conducting thorough assessments, identifying strengths and areas for development, and developing innovative solutions to address complex needs. You will also contribute to the design and implementation of new initiatives to improve outcomes for children and young people.
To excel in this role, you will need to possess a Social Work degree, completed ASYE, and registration with Social Work England. You should also demonstrate excellent communication and collaboration skills, with the ability to work effectively with diverse stakeholders.
Key Responsibilities**
- Develop and implement care plans that address complex needs and promote independence
- Collaborate with multi-agency partners to ensure seamless delivery of services
- Monitor and evaluate progress, making adjustments as needed
- Identify and escalate safeguarding concerns in a timely manner
Salary and Benefits**
We offer a competitive salary package, including £39,951 - £46,041 per annum, plus a £2,500 recruitment and retention allowance. You will also receive a generous pension scheme, annual leave up to 27 days, and opportunities for career progression and professional development.
