Community Nursing Team Deputy Manager

Cornwall Partnership NHS Foundation Trust is seeking a Community Nursing Team Deputy Manager to join our team. As a key member of our staff, you will be responsible for managing a clinical caseload within a community nursing team, providing personalized support plans, and promoting informed choice.
This role requires strong leadership skills, conflict resolution abilities, and advanced computer skills, including proficiency in Microsoft Office and spreadsheets.
You will work collaboratively with patients, carers, and healthcare professionals to achieve safe, effective admission avoidance and timely discharge from hospitals and other care settings.
Main Duties:
- Manage a clinical caseload within a community nursing team
- Develop and implement personalized support plans
- Collaborate with multidisciplinary teams to achieve safe, effective admission avoidance and timely discharge
- Promote informed choice and independence
- Lead by example, demonstrating a commitment to excellent patient care
Person Specification:
- Current professional registration - RN (Adult)
- Evidence of recent relevant professional development
- A degree in nursing
- Leadership skills and conflict resolution abilities
- Advanced computer skills, including proficiency in Microsoft Office and spreadsheets
