Mental Health Practitioner Lead

Estimated Salary: £45,000 - £55,000 per annum.

About the Role

We are seeking an experienced Mental Health Practitioner Lead to join our team. As a key member of our clinical governance team, you will play a crucial role in developing and implementing quality monitoring processes to improve service delivery.

  • You will lead research projects and audit activity to identify areas for improvement and implement changes to enhance service outcomes.
  • You will work closely with staff to develop and implement care pathways that reduce the risk of further crisis developing.
  • You will provide high-quality patient care in various settings, including homes, Fieldview, and hospitals.
Key Responsibilities

The successful candidate will be responsible for:

  • Liaising with community-based services and resources to facilitate the delivery of best possible care for clients and their families.
  • Discussing complex issues surrounding risk management and planning care accordingly.
  • Discharge planning and developing clear care pathways.
  • Providing telephone support and making effective decisions that impact client outcomes.
  • Participating in on-call rotas and covering 7 days a week, 24 hours a day.
Requirements

To be considered for this role, you must have:

  • A degree in a relevant field or equivalent experience.
  • Proven experience in mental health practice and leadership.
  • Excellent communication and interpersonal skills.
  • Able to work flexibly and respond to changing priorities.
