As a Clinical Medication Optimisation Lead with Somerset NHS Foundation Trust, you will be responsible for developing and implementing strategies to improve patient safety and reduce medication errors. This is an exciting opportunity to contribute to the development of clinical pharmacy services and make a real difference in the lives of patients.

You will work closely with healthcare staff to identify areas for improvement, develop and implement new initiatives, and evaluate their impact on patient care. Your responsibilities will include reviewing prescriptions for clinical appropriateness, identifying potential drug interactions, and providing recommendations to enhance patient outcomes.

To succeed in this role, you will require a Master's degree in Pharmacy, full registration with the General Pharmaceutical Council (GPhC), and completion of or expectation to complete a postgraduate Diploma or MSc in Clinical Pharmacy. Previous hospital experience and commitment to continuous professional development are essential.

We offer a competitive salary of £65,000 - £85,000 per annum, depending on experience, plus a range of benefits including flexible working arrangements, NHS pension scheme, and generous annual leave allowance.
