About the Job

  • We are seeking a Community Healthcare Worker to join our team, providing person-centered care to our clients.
  • The successful candidate will be responsible for supporting clients with daily living tasks, medication management, and social activities.

Job Description

  1. Deliver personalized care plans, tailored to meet the unique needs of each client.
  2. Provide physical assistance with daily living tasks, such as bathing, dressing, and grooming.
  3. Administer medication, as prescribed by healthcare professionals.
  4. Prepare meals and assist with food preparation, promoting healthy eating habits.
  5. Engage in social activities with clients, reducing feelings of loneliness and isolation.

Requirements

  • A valid UK driving license and use of own vehicle (for some roles).
  • Able to work a variety of shifts, including days, evenings, and weekends.
  • Flexible and willing to cover nights and alternate weekends.
  • Confident English speaker with excellent communication skills.
