Care Services Coordinator Lead

The Amber Mace Group is seeking a seasoned Care Coordinator to oversee the coordination and delivery of high-quality care services in Winchester. In this pivotal role, you will lead the planning and implementation of care plans, ensuring seamless transitions and optimal outcomes for Service Users.
You will leverage your expertise to develop and manage effective rotas, aligning staffing levels with service user needs. This will include monitoring attendance, addressing any gaps or discrepancies, and implementing necessary adjustments to meet changing demands.
Responsibilities:
- Develop and implement care plans tailored to individual Service User needs.
- Maintain accurate records and reports to track progress and address any concerns.
- Collaborate with colleagues to resolve issues, escalate incidents when necessary, and provide guidance on best practices.
Expected Qualifications:
- Demonstrated experience in care coordination, ideally within the homecare sector.
- Proven ability to effectively manage multiple priorities, delegate tasks, and ensure seamless service delivery.
- Exceptional communication and interpersonal skills.
Estimated Salary Range: £34,500 - £40,000 annually, based on qualifications and experience.
