This is an exciting opportunity to join our team as a Recovery Journey Specialist at Devon Partnership NHS Trust. We are seeking a highly motivated and experienced individual to support individuals in their recovery journey through secure care and into the community. As a key member of our Community Forensic Team, you will be responsible for providing support to both inpatient and community-based service users, and having an active involvement in the delivery of high-quality mental health services. Your expertise and passion for mental health recovery will make a significant impact on the lives of those you work with.

Responsibilities:
- To contribute to the assessment and care planning process.
- To support the Care Coordinators with the assessment and care planning process.
- To use specific standardised assessment tools, including carers assessments and physical health assessments under supervision.
