About the Role

We are seeking a highly motivated and dedicated individual to join our team as an Assistant Practitioner at Nottingham CityCare Partnership CIC. As an Assistant Practitioner, you will play a vital role in providing high-quality, patient-centered care to individuals in our community settings.

Key Responsibilities
  • Work alongside Nurses, Physiotherapists, Occupational Therapists, and Rehabilitation Support Workers to provide assessment and treatment interventions to individuals in our community settings.
  • Support the work of clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies, and procedures.
  • Participate actively in the service's training strategy, including in-service training and multi-agency training.
  • Maintain a safe working environment and be aware of the necessity to comply with health and safety regulations.
  • Have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not disclose information to unauthorized parties.
Person Specification

We are looking for an individual who possesses the following skills and qualifications:

  • Knowledge of professional area procedures and practices (nursing, physiotherapy, or occupational therapy)
  • Knowledge of Health and Social Care provision
  • Knowledge of need and risk in relation to Health Care provision in the community
  • Understanding the concepts of assessment and care planning
  • Awareness of Rehabilitation and/or Nursing Care process
  • Awareness of promoting/enabling independence and self-care
  • Awareness of a range of activity of daily living equipment and adaptations
  • Effective communication skills (verbal and written) and good interpersonal skills
  • Ability to prioritize and plan workload
  • Ability to follow the Health Care Support Workers code of conduct policy and maintain professional boundaries
  • Ability to recognize the need for help and support from other professionals
  • Ability to contribute to the effective working of a team
Requirements

We require an individual who:

  • Is educated to Foundation degree level or equivalent
  • Has experience of multi-disciplinary team working in a Health Care, Social Care, or Voluntary Care setting
  • Has experience of managing own workload in a team setting
  • Has experience of working with individuals and/or groups within a Health Care, Social Care, or Voluntary Care setting
  • Has experience of working with individuals who have long-term conditions
  • Has experience of record keeping and accurate data recording
