Our well-established Integrated Community Stroke Service is dedicated to delivering rehabilitation across various community environments. Services are currently available from 8am to 5pm, five days a week, including Bank Holidays, with potential weekend work in the future.

Key Responsibilities

As an Advanced Physiotherapist within the Integrated Community Stroke Rehabilitation service, you will play a crucial role in providing leadership and guidance to staff regarding the delivery of specialized community stroke rehabilitation and ongoing case management. Your responsibilities will include:

- Utilizing advanced clinical reasoning, critical thinking, and reflective practice to support the assessment, treatment, and management of stroke survivors with complex needs.

- Collaborating closely with the multidisciplinary team (MDT), effectively communicating with clients, their families, and caregivers, while supervising and mentoring other staff as necessary.

- Empowering patients within the community to achieve optimal independence and self-care following a stroke.

- Ensuring that the Community Stroke Rehabilitation service adheres to accreditation standards and works towards meeting the guidelines set by the Royal College of Physicians (RCP) and the National Institute for Health and Care Excellence (NICE).

This role offers the successful candidate the chance to work collaboratively within an integrated multidisciplinary team, providing specialized interventions to patients referred to the Community Stroke Rehabilitation services. The MDT includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Clinical Neuro-Psychology, Assistant Practitioners, and Rehabilitation Support Workers.

The service facilitates the timely discharge of stroke patients into their home environment, providing specialized rehabilitation in the community. You will play a key role in supporting timely discharges from inpatient settings by collaborating closely with our partners. The team focuses on specific patient goals while also addressing the needs of their caregivers. You will manage complex cases throughout the rehabilitation pathway.

We are looking for an enthusiastic and motivated Advanced Physiotherapist capable of managing intricate cases and adapting to various work environments.

Qualifications and Experience

- Recognized Physiotherapy Degree Qualification

- Health Professions Council License to Practice

- Significant experience working at a senior level in stroke/neuro rehabilitation

- Knowledge of RCP and NICE guidelines for stroke rehabilitation

- Experience in leading and managing a team

- Valid UK driving license and access to a vehicle for work purposes
