Job Summary:

This is a challenging and rewarding role that requires a high level of clinical expertise and leadership skills. As a Rehabilitation Case Manager, you will be responsible for managing a caseload of patients with traumatic brain injuries and other neurological conditions.

Key Responsibilities:

  • Manage own caseload to assess and implement rehabilitation packages for individuals and groups
  • Provide education and support to families and carers of patients
  • Provide clinical leadership through case management and contribute to the rehabilitation and care of patients
  • Develop and implement evidence-based approaches for the assessment and management of traumatic brain injury and other neurological conditions
  • Work collaboratively as part of an interdisciplinary team to achieve the best possible outcomes for patients

Requirements:

  • High level of clinical expertise in rehabilitation and case management
  • Strong leadership and communication skills
  • Ability to work collaboratively as part of a team
  • Flexibility to work across seven days, including weekends
