We are seeking an experienced and skilled Senior Heart Failure Specialist Nurse to join our team at York and Scarborough Teaching Hospitals NHS Foundation Trust. Job Summary


The successful candidate will have expertise in managing cardiac patients, particularly those with heart failure. They will work collaboratively across primary and secondary care interfaces to ensure seamless delivery of care, utilizing their knowledge of NMP qualifications and advanced clinical assessment skills.


Main Responsibilities


This role involves:



  • Evaluating and developing individualized management plans for heart failure patients based on current evidence and guidelines
  • Providing comprehensive assessment and treatment services across various settings, including acute assessments, community clinics, and home visits
  • Collaborating effectively with colleagues from diverse backgrounds, fostering open communication and promoting a culture of inclusivity
  • Developing new strategies for managing complex patient populations, driving innovation, and enhancing quality improvement initiatives

Your Requirements


To succeed in this role, you must possess:



  • Professional qualification in Heart Failure management (e.g., ENB 124 or equivalent)
  • Non-medical prescribing qualification
  • Excellent communication and interpersonal skills, enabling effective collaboration with patients, carers, and multidisciplinary teams
  • Ability to prioritize tasks, manage workload efficiently, and demonstrate flexibility and problem-solving skills
  • Car driver with full license and access to own transport for travel between sites
