About the Role

We are seeking an experienced Heart Failure Specialist Nurse to join our team at Clementhorpe Health Centre, offering services across the Vale of York at several sites.

This role requires the ability to transport yourself and equipment across the locality during a normal working day. You will need access to a vehicle that can be used for business purposes and hold a full and valid driving licence, as the role involves travel to numerous addresses that cannot be easily accessed by public transport.

Key Responsibilities

Assessment and Management of Heart Failure Patients

You will be responsible for assessing and managing heart failure patients, initiating an evidence-based management plan according to individual need.

This will involve working across all services within the team, including acute assessment and delivery of IV diuretics, community clinic services, and home visiting where appropriate.

Collaboration and Communication

You will work collaboratively with colleagues across the acute, community, and primary care networks, developing new and exciting ways of managing complex patients and leading in research and innovation.

Essential Skills and Qualifications

Excellent communication and interpersonal skills are essential, as is the ability to organise your own workload and prioritise effectively.

You will need to be able to work autonomously, with evidence of flexibility, innovation, and problem-solving.
