Job Summary

We are seeking an experienced Cardiac Care Specialist to join our team at York and Scarborough Teaching Hospitals NHS Foundation Trust. The successful candidate will have a strong background in heart failure management and clinical assessment skills.

Main Responsibilities

The ideal candidate will be responsible for assessing and managing heart failure patients, developing individualized care plans, and working collaboratively with colleagues across the acute, community, and primary care networks.

About the Role

This is an exciting opportunity for a motivated and experienced nurse to lead in the management of heart failure patients and contribute to the development of new and innovative ways of delivering care.

Requirements

The successful candidate will have a professional qualification in heart failure management, a degree-level specialist training, and a Non-Medical Prescribing qualification. They will also have experience of managing cardiac patients at a senior level and a strong understanding of primary care and the need to work collaboratively across the primary/secondary care interface.
