We are looking for a skilled Social Worker to join our team in Greater Manchester. As a qualified Adult's Care Manager, you will work in a dynamic environment with a highly motivated and passionate team. Your primary responsibility will be to complete assessments for clients and carers and provide care coordination in line with the Trust Care Programme Approach (CPA) guidance and policies.
