Social Work Role at Bury Adult Care Services

We are seeking a highly motivated and experienced Social Worker to join our team at Bury Adult Care Services. This vital role demands dedication to ensuring the safety and wellbeing of our service users.

  1. Thorough assessments of referrals to guarantee effective support.
  2. Personalized contact with service users meeting CAD Triage criteria to ensure their safety and wellbeing.
  3. Liaison with professional services, including CMHT, OPCMHT, Police, Hospital, and GP's, to address complex cases.
  4. Locating and promoting community resources and support groups.
  5. Initial Needs Assessments to secure suitable housing options.
  6. Home visits as necessary to maintain a high level of support.
  7. Risk recognition and mitigation strategies to ensure optimal outcomes.
  8. Supporting Enquiry Officers in responding to concerns and managing calls received.
  9. Notifying EDT and signposting to relevant teams when required.

About the Position:
