Job Summary

This Wellbeing Support Specialist role is an excellent opportunity to contribute to the health and wellbeing of vulnerable adults in our community. As part of our CAD Team, you will work collaboratively with colleagues and partners to deliver high-quality support and interventions that promote independence and dignity.

Key Responsibilities:
  1. Delivering social work interventions, including assessments, plans, and reviews.
  2. Collaborating with internal and external stakeholders to achieve shared goals and improve service user outcomes.
  3. Engaging with community groups and organizations to identify and access relevant resources and support services.
  4. Providing case management and coordination to ensure seamless service delivery and minimize risk.
  5. Staying up-to-date with best practices and maintaining expertise through ongoing professional development and training.
About You

Salary: £60,000 - £70,000 per annum
