About the Position

Tripod Partners is looking for an experienced Social Worker to join its CAD Team. As a key member of the adult care services team, you will be responsible for ensuring the safety and wellbeing of service users.

Main Responsibilities

Your key duties will include:

  1. Evaluating referrals sent to Bury Adult Care Services to identify potential risks and provide support.
  2. Developing relationships with service users who meet the criteria for CAD Triage to guarantee their safety and wellbeing.
  3. Working with various professional services, including CMHT, OPCMHT, Police, Hospital, and GP's, to create comprehensive support plans.
  4. Locating and accessing relevant services, such as Food Banks, BAME Groups, Men/Women/LGBT+ Support Groups, to enhance service user outcomes.
  5. Conducting initial Needs Assessments to facilitate the placement of service users in suitable housing.
  6. Visiting service users in their homes when necessary to assess their needs and offer targeted support.
  7. Recognizing and addressing risk situations, taking prompt action to prevent potential harm.
  8. Supporting Enquiry Officers by providing guidance on CAD Triage procedures and directing service users to relevant teams.
  9. Responding to EDT Notifications by carrying out further investigations or referring service users to relevant teams.
  10. Returning SF1 forms that do not meet SG Team criteria to CAD Triage for management.
Qualifications and Experience

To be considered for this role, you must possess:

  • A degree in Social Work.
  • Experience working in adult social care.
  • A commitment to improving the lives of vulnerable adults.
