About the Role

As a Mental Health Community Care Coordinator, you will be responsible for assessing, planning, and reviewing the care needs of individuals, ensuring their safety and well-being. You will work closely with healthcare professionals, families, and carers to develop and implement individualized care plans.

This role requires a strong understanding of the Care Program Approach and the ability to complete and update risk assessments. You will also be expected to demonstrate anti-discriminatory practice and promote the rights of vulnerable adults.

Key Responsibilities
  1. Care co-ordination in line with the Care Program Approach.
  2. Completing and updating risk assessments.
  3. Working in line with the Care Act 2014, Mental Capacity Act (2005), and Mental Health Act (1983).
  4. Assessment, support planning, reviews, and safeguarding within the meaning of the Care Act 2014.
  5. Demonstrating anti-discriminatory practice and promoting the rights of vulnerable adults.
  6. Demonstrating an ability to collate information and write reports of a standard fit for legal processes such as Mental Health Tribunal's and Hospital Managers Hearings.
  7. Fulfilling the duties of an Approved Mental Health Professional / Best Interests Assessor or being willing to work towards gaining the associated qualifications.

The estimated annual salary for this role is around £35,000-£45,000, dependent on experience. We offer a range of benefits, including regular social work peer support groups, reflective practice groups, mindfulness, and other team activities.
