About the Role
">We are seeking a highly skilled Social Worker to join our team as a Care Coordinator in our Community Mental Health Team. As a key member of our integrated, multi-disciplinary team, you will be responsible for conducting assessments, identifying health and social care needs, and coordinating care packages for our service users.

Requirements

- Degree in Social Work (DipSW, BA, MA) and registration with the HCPC;">
- Experience in conducting assessments, commissioning care packages, and safeguarding vulnerable adults;">
- Excellent communication and interpersonal skills, with the ability to work flexibly;">
- A commitment to upholding individuals' human rights and promoting anti-oppressive practice.">

- An estimated salary of £30,000 - £40,000 per annum, depending on experience;">
