Community Health Nurse

We are committed to delivering high-quality care to our patients and communities. As a Community Health Nurse, you will play a vital role in supporting the nursing needs of housebound patients in and around Ramsgate.
Our team provides valuable care, empowering patients to manage their long-term conditions and maximize their independence. This enables patients to stay safe and well at home or in their chosen environment.
We offer a range of services, including wound management, palliative and end-of-life care, and symptom control. Our aim is to ensure that our patients have access to the care they need, when they need it.
Key Responsibilities- Support the nursing needs of housebound patients in Ramsgate and surrounding areas.
- Enable patients to manage their long-term conditions and maximize their independence.
- Provide wound management, palliative and end-of-life care, and symptom control services.
- Registered Healthcare Professional with relevant Professional body (1st Level Nurse/NMC).
- Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.
- Awareness of professional issues and recent developments in the NHS.
- Full driving licence and access to a vehicle.
