About the Role
Kent Community Health NHS Foundation Trust is seeking a skilled Healthcare Service Coordinator to join our team. As a Healthcare Service Coordinator, you will play a vital role in supporting patients with complex needs in the community.
Main Responsibilities
- Assessing and planning care needs for patients
- Liaising with other team members to ensure safe and high-quality care
- Providing patient-focused care in a community-based setting
Requirements
- Minimum of 2 years' experience as a Senior Healthcare worker
- Experience of working as part of a team and using Microsoft Office and various IT packages
- Awareness of professional issues and recent developments in the NHS and Primary Care
