Job Summary:

We are seeking a highly skilled and compassionate Carer Support Specialist to join our Family Carer Team. As a key member of our team, you will work closely with Carers to promote their wellbeing and support them to continue managing their caring role.

Key Responsibilities:

  • Conduct Carers Needs Assessments to identify and address their support needs
  • Support Carers to plan their caring role and access relevant services
  • Facilitate access to the Carer's Emergency Card and Carers Direct Payment
  • Work collaboratively with community-based services to meet the needs of Carers
  • Provide strength-based, person-centred, and relational support to promote independence, wellbeing, inclusion, and choice and control

Requirements:

  • Experience of working with Carers and understanding of their needs
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Knowledge of relevant policies and procedures
