About the JobIn this role, you will be responsible for assessing patients' needs, developing and implementing personalized rehabilitation plans, and collaborating with multidisciplinary teams to achieve patient goals. Your clinical expertise and experience in managing healthcare assistants will be invaluable in this position.
You will work closely with colleagues in both acute and community hospitals to ensure smooth transitions of care and facilitate timely discharges from hospital. A valid UK driving license, access to a vehicle for work purposes, and flexibility to respond to need at short notice are essential requirements for this role.
Requirements- Professional registration with the Health and Care Professions Council (HCPC)
