Clinical Leadership Specialist

About the Role: We are seeking an enthusiastic and motivated individual to join our Community Specialist Palliative Care Team as an Advanced Clinical Practitioner. The post holder will provide advanced clinical leadership, exercising independent judgment to assess, investigate, diagnose, plan, implement, and evaluate the clinical care and management of patients within their care.
Key Responsibilities:
- Exercise independent judgment in assessing, investigating, diagnosing, planning, implementing, and evaluating patient care.
- Provide advanced clinical leadership, mentoring, and clinical supervision to ensure a consistent quality-driven delivery of service.
- Work within local and nationally agreed frameworks to develop and improve systems and processes that facilitate patient flow, timely discharge, and preventing hospital admission.
Requirements:
- Experience in developing and improving systems and processes that facilitate patient flow, timely discharge, and preventing hospital admission.
- Community experience working within a multidisciplinary led environment.
- Able to work across boundaries within primary care and offering support to secondary care.
