About the Role

We are seeking a highly skilled and experienced Nurse Assessor to join our team at NHS Midlands and Lancashire Commissioning Support Unit. As a key member of our Personalised Healthcare Commissioning team, you will play a vital role in supporting the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).

Key Responsibilities
  • Conduct comprehensive person-centred assessments to determine CHC and FNC eligibility.
  • Work collaboratively with individuals and their representatives to ensure a high-quality service is delivered.
  • Manage a defined caseload of patients with complex health needs.
  • Facilitate and contribute to multi-disciplinary assessments and patient reviews.
  • Support the Personalised Healthcare Commissioning team to ensure workstream plans are fulfilled and supervised effectively.
  • Provide specialist advice and guidance on functional and information matters relating to the service.
  • Contribute to performance improvement within the team, ensuring services are delivered to a high standard.
Requirements
  • Experience of working in a health environment.
  • Education to degree level in a relevant subject or equivalent level of qualification or experience in a specialist area.
  • Healthcare Professional Registration.
  • Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the continuing healthcare process and MDT working.
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales.
