Role Overview: As a Senior Practitioner in Memory Care, you will take on the pivotal role of care coordinator, overseeing the assessment, planning, and implementation of care for a designated patient demographic.
Key Responsibilities:
- Act as a Change Agent, proactively enhancing practice standards.
