Role Overview: As a Senior Practitioner in Memory Care, you will take on the pivotal role of care coordinator, overseeing the assessment, planning, and implementation of care for a designated patient demographic.

Key Responsibilities:

  • Act as a Change Agent, proactively enhancing practice standards.
  • Foster relationships and strengthen communication among various stakeholders.
  • Serve as the primary care coordinator for a specific group of patients within the broader community team.
  • Ensure that tailored interventions are devised and executed based on assessed needs.
  • Promote a constructive nurse-patient rapport and engage with caregivers and family members as necessary.

Documentation and Standards: You will be responsible for maintaining accurate care plans, ensuring they are up-to-date and that record-keeping standards are consistently met.

Additionally, you will play an active role in auditing and upholding the quality of record-keeping for this patient group.
