About the Role
The North Staffordshire Combined Healthcare Trust is seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at the Darwin Centre. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to younger people and enhancing their mental health and wellbeing.
Key Responsibilities
- To act as a role model and provide specialist advice to junior staff, ensuring the delivery of high-quality care and supervision of others.
- To participate in the assessment of care needs and the development, implementation, and evaluation of individual care plans, in collaboration with the multidisciplinary team.
- To take responsibility for setting, maintaining, and evaluating standards of care throughout the 24-hour period, in conjunction with the Team Manager.
- To liaise with all members of the multidisciplinary team and outside agencies to implement patient care, ensuring effective integration and collaboration.
- To contribute to the development of innovative practice and ideas, and participate in departmental audit as requested by the Team Manager.
