We are seeking a highly motivated and experienced Mental Health Practitioner to join our team at North Staffordshire Combined Healthcare Trust. As a key member of our Assessment and Triage Team, you will play a vital role in ensuring that our service users receive high-quality care and support.
Key Responsibilities- Conduct thorough assessments of service users to identify their needs and develop effective care plans.
- Work collaboratively with our multi-disciplinary team to provide comprehensive support and interventions.
- Act as a Duty Professional on a rotational basis, providing 24/7 support and interventions to service users.
- Liaise with other professionals to ensure high-quality, gold-standard care and support.
We are looking for a qualified and experienced Mental Health Professional who possesses the following skills and qualifications:
- A registered Mental Health Nurse, Occupational Therapist, or Social Worker with a minimum diploma level or equivalent qualification.
- Experience of working therapeutically in community settings.
- Knowledge of various approaches to mental health assessment.
