About the Role

We are seeking a highly motivated and experienced Mental Health Practitioner to join our Assessment and Triage Team at North Staffordshire Combined Healthcare Trust. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals in our local community.

Key Responsibilities
  • Conduct thorough assessments of individuals referred to our service, using a multi-disciplinary approach to ensure effective decision-making.
  • Participate in the triage and management of referrals, working closely with our multi-disciplinary team to ensure consistency and quality.
  • Act as a Duty Professional on a rotational basis, providing support and interventions to individuals in need, including home visits and follow-up appointments.
  • Liaise with other professionals to ensure high-quality, Gold Standard care and support.
Person Specification

We are looking for a qualified and experienced Mental Health Practitioner who possesses the following skills and qualifications:

  • A registered Mental Health Nurse, Occupational Therapist, or Social Worker with a minimum diploma level or equivalent qualification.
  • Experience of working therapeutically in community settings.
  • Knowledge of various approaches to mental health assessment.
