About the Role

We are seeking a highly skilled and experienced Clinical Assessor to join our team at the Hampshire and Isle of Wight Integrated Care Board (ICB). As a Clinical Assessor, you will play a vital role in supporting individuals across all ages who require ongoing care.

Key Responsibilities
  • Co-ordinate and deliver needs/eligibility assessments, case management, and reviews of patients under the relevant frameworks for NHS Continuing Healthcare, Funded Nursing Care, and Fast Track.
  • Work closely with community services and care providers to develop systems and processes to facilitate a smooth assessment and referral process.
  • Communicate complex information and administrative matters to patients, their families, and other professionals.
  • Produce accurate, detailed, and comprehensive reports.
  • Assess and accurately interpret health and wellbeing needs working to Department of Health guidance for all those identified with registered nursing or healthcare needs.
  • Develop, monitor, and review programmes of care to meet identified needs.
Person Specification
  • Registered Health or Social Care Professional with current registration with a professional body (NMC, HCPC, or SWE).
  • Knowledge of recent developments in the NHS.
  • Working knowledge of IT systems.
  • Clinical assessment skills.
