Clinical Assessor

We are seeking a highly skilled Clinical Assessor to join our team at Hampshire and Isle of Wight ICB (104). As a Clinical Assessor, you will play a vital role in supporting individuals across all ages who require ongoing care.
Key Responsibilities- Work within the NHS All Age Continuing Care team with responsibility for co-ordination and delivery of needs/eligibility assessment, case management and review of patients under the relevant frameworks for NHS Continuing Healthcare, Funded Nursing Care, Fast Track.
- Assess and accurately interpret health and wellbeing needs working to Department of Health guidance for all those identified with registered nursing or health care needs.
- Develop, monitor and review programmes of care to meet identified needs, including the giving of specialist advice to patients, carers and members of health and social care teams.
- Maintain constructive relationships with a broad range of internal and external stakeholders.
- Communicate information and issues, including briefings and reports, to job manager, strategic lead/strategic manager as appropriate.
- Registered Health or Social Care Professional with current registration with professional body NMC, HCPC or SWE.
- Knowledge of recent developments in the NHS.
- Working knowledge of IT systems.
- Clinical assessment skills.
