Home Care Coordinator for Older Adults

Job Overview
We are seeking a Home Care Coordinator to join our team at Your Housing Group, a leading social landlord in the North. As a key member of our Older People Services team, you will play a vital role in supporting older adults living in our retirement schemes.
About the Role
This is an exciting opportunity to work with a dedicated team who are passionate about providing high-quality care and support to older adults. As a Home Care Coordinator, your primary responsibility will be to coordinate and deliver home care services to residents living in our retirement schemes.
Key Responsibilities
- Coordinate home care services for residents, including arranging visits and managing service delivery
- Work closely with our Older People Services team to ensure seamless delivery of care and support services
- Build strong relationships with residents, their families, and external stakeholders to ensure effective communication and collaboration
- Maintain accurate records and reports to monitor service delivery and resident outcomes
- Contribute to the development and implementation of care plans and strategies to improve resident outcomes
What We Offer
In return for your hard work and dedication, we offer a competitive salary of £13,104 per annum, plus 25 days annual leave (pro-rata for part-time hours) and Bank Holidays. You will also have access to our wellbeing, social, collaboration, and meeting space in Birchwood, Warrington.
