Hospital Discharge Care Coordinator

Salary: £28,000 - £40,000 per annum
Ackerman Pierce Ltd is seeking an experienced Discharge Practitioner to join their team in Somerset. This role involves supporting adults in the community, delivering care and support services, and giving people control over their care and support.
About the Role:
- Support adults in their community to remain independent for as long as possible.
- Deliver care or support through high-quality, joined-up social care, health, and wellbeing services.
- Support people out of hospitals in a timely manner and prevent admissions.
Requirements:
- HCPC Registration
- 1-2 years post-qualifying experience within the UK Social Care industry
- Enhanced DBS/CRB (we can obtain for you)
- Eligibility to work in the UK
