Care Practitioner

Ackerman Pierce Ltd is seeking an experienced Care Practitioner to join their Hospital Discharge team in Somerset. As a key member of the team, you will play a vital role in supporting adults in their community to maintain independence.
The ideal candidate will have 1-2 years of post-qualification experience within the UK Social Care industry and hold HCPC Registration. Enhanced DBS/CRB clearance is also required. In addition to a competitive salary, you can expect a range of benefits, including access to CPD opportunities and guaranteed weekly payments.
Salary: £40,000 - £60,000 per annum
Key Responsibilities- Support adults in their community to remain independent for as long as possible
- Deliver care or support through high-quality, joined-up social care, health, and wellbeing services
- Prevent admissions by using people's own and community assets to enable them to return home
- HCPC Registration
