Social Worker Opportunities in Somerset Council

We are currently seeking a qualified social worker to join our team as a hospital discharge social worker in Somerset Council. This is a challenging but rewarding role that requires strong communication and organisational skills.

Job Description:

The successful candidate will be responsible for working with vulnerable adults within a hospital setting, developing care plans, and managing complex cases to ensure safe and timely discharges. They will also be required to liaise with other professional bodies and arrange safe and timely discharges for service users.

Requirements:
  • HCPC Registration
  • 1 year post qualifying experience within UK Social Care industry
  • Enhanced DBS/CRB (we can obtain for you)
