Clinical Care Coordinator

As a qualified social worker in the hospital discharge team, you will be working with vulnerable adults within a hospital setting. This role involves developing care plans, managing complex cases, and liaising with other professional bodies to ensure safe and timely discharges for service users.
Job Requirements:- HCPC Registration
- 1 year post qualifying experience within UK Social Care industry
- Enhanced DBS/CRB (we can obtain for you)
We are looking for an experienced social worker who is able to work independently and as part of a well-structured organised team. In return, we offer a competitive salary of £35,000 - £45,000 per annum, depending on experience.
