About the Role
This exciting opportunity requires a candidate with extensive experience in care management, preferably within a similar environment. Your primary responsibility will be to provide strategic leadership and direction to our care team, ensuring that all aspects of care delivery meet the highest standards of quality and excellence.
- Developing and implementing comprehensive care plans tailored to individual needs.
- Maintaining accurate records and reporting on care delivery and outcomes.
- Providing training and support to care staff to enhance their skills and knowledge.
- Fostering effective communication and collaboration with healthcare professionals and other stakeholders.
Our Ideal Candidate
To succeed in this role, you will require:
- Minimum 2 years' experience in a care management position.
- Strong IT skills, including proficiency in Microsoft Office.
- Excellent communication and interpersonal skills.
- A valid driving license and access to a vehicle.
What We Offer
In return for your expertise and dedication, we offer a competitive salary range of £45,000 - £60,000 per annum, depending on experience, as well as access to ongoing training and development opportunities.
