As a Care Coordinator for our Sherwood Lodge service in Mansfield, you will play a vital role in ensuring the best possible outcomes for our residents. This includes working closely with multidisciplinary teams to develop and implement personalized care plans, as well as providing guidance and support to help individuals achieve their goals and aspirations.

The successful candidate will have experience working in a similar setting and possess excellent communication and interpersonal skills, with the ability to build strong relationships with colleagues, families, and other stakeholders. You will also be passionate about empowering individuals with learning disabilities to live fulfilling lives and make informed decisions about their care.

In return, we offer a competitive salary of £22,000 per annum, plus benefits including flexible working hours, regular coaching and appraisal, and opportunities for career development and progression.
