Role Description

We are looking for a skilled Social Care Transition Coordinator to join our Preparing for Adulthood team in Somerset. As a coordinator, you will progress cases of service users from Children Services to Adult Services, work jointly with SEN Coordinators, and attend meetings to set up holistic support plans.

The ideal candidate will have 1-2 years of post-qualifying experience in the UK Social Care industry, be registered with the HCPC, have an enhanced DBS/CRB, and be eligible to work in the UK. You will also need to be able to work closely with families and service users, promoting their views and wellbeing in all interventions.
