Location: Somerton, Somerset.

We are currently seeking a compassionate, caring, and experienced care coordinator to join our growing team at BooCare.

**About BooCare**

BooCare is a small, family run business, led by a Nurse Consultant, where we believe that care matters, and we are committed to providing the highest quality of home care in Somerset so that our service users can remain in their own homes. We cover areas including Somerton, Langport, Street, Glastonbury, and the surrounding small villages.

We work alongside families and individuals to help our service users stay in their own care for as long as they are able. **The Role**

This role is part time (22.5 hours) per week and will require you to work a mix of shifts per week (information below) and **alternate **weekends.

Our current shifts include (_including an unpaid 30-minute lunch break_):
Early (07:00-15:00)

Late (14:00- 22:00)

The role will be predominately office based, however you may be require to cover any staff sickness on your shift. There will also be a requirement to work 5 hours of care per week. It is your choice as to when and how you use those 5 hours over the week. For instance, this can be used for service user reviews or supporting staff on visits.

The Care Coordinator will work aside our Supervisor, Care Co Coordinators and Office Manager.

Care Coordinator Duties and Responsibilities will include:
Staff
- Manage and oversee a small team of staff.
- Conducting and participating in staff training.
- Leading the shift.
- Conduct visits to supervise and assess staff.

Service Users
- Creating and maintaining relevant and personalised care plans for service users and ensuring they are regularly reviewed.
- Carrying out the first visit with new service users and providing information to staff on their care plan and needs.
- Providing a ‘shadow’ system with staff on the first visit to a new service user.
- Undertaking service user reviews and collecting feedback through quality reviews to ensure Service User satisfaction.
- Reporting any issues, concerns, or safeguarding concerns to higher management immediately.
- Effective communication with staff, service users, family members and multi-disciplinary teams.
- Maintaining compliance with all legal and company requirements.

**Qualifications**

Required:

- Minimum of 2 Years’ Experience In Domiciliary Care.
- Ability to work a mix of shift patterns and alternate weekends.
- Full UK Driving Licence and access to own vehicle.
- Good Knowledge with IT systems, including the use of Microsoft Office.
- Excellent communication skills, both written and verbal.
- Basic clerical skills.
- Ensuring you are compassionate in each task you undertake.
- Excellent and polite phone etiquette.
- Excellent organisational and interpersonal skills.
- Ability to remain calm in difficult or challenging situations and proactively find solutions.

Preferred:

- Relevant Certifications in Care.
- Experience as Care Coordinator or another relevant role.

**Pay Rates**:

- **£12.50- Monday
- Friday**:

- **£13.25- Saturday and Sunday**:

- **£21.50- Bank Holiday**

Any mileage used whilst on shift will be paid at 0.45p per mile _(Please be aware this _**does not**_ _include any travel to or from work)._

Additional Information:
**Probationary Period**: There will be a three month probation period.

**Annual Leave**: Equivalent to 28 days pro rata per year. Annual leave period commences 1st April and ends on 31st March.
