Trust In Care is currently recruiting for a **Care Coordinator** to join its expanding team.

**Office Location**: West Bromwich

**Hours**: Monday - Friday 9am - 5pm with occasional weekends

Trust In Care is a Home Care company providing high quality care to individuals within the comfort of their own homes. Our care teams provide person centered care unique to each individual we care for. Our services are provided across Sandwell and surrounding areas.

You will be joining a friendly and genuinely caring company that work as a team and provide flexibility with work life balance. Our services are available to people we support from 6.30am - 10.30pm 7 days per week.

**The Role**:

- Assisting the Registered Manager and Care Coordinators with day to day running of the business
- Care Planning, Risk Assessments for existing and new clients
- Coordinating care for given areas and providing assistance to carers
- Management of your own area team of care staff
- Working with all stakeholders across health and social care to ensure people we care for are safe and well in their own environment

**About you**:

- Minimum 4 years experience of working in the care sector
- Minimum - Level 3 Adult Health & Social Care Qualified
- Previous experience of working in similar role and providing person centered care and support
- Knowledge of care planning and carrying out risk assessments in the home care setting
- Good people management skills
- Flexible and and willingness to go the extra mile

**Salary**: £22,297.53-£23,500.00 per year
