**Job Description:**

We are seeking a highly skilled and experienced Lead Care Coordinator to join our team at Care UK. This is an excellent opportunity for individuals who have a passion for delivering high-quality care and supporting our residents to live fulfilling lives.

**Key Responsibilities:

  • Coordinating care delivery, ensuring seamless transitions between shifts and departments
  • Developing and implementing individualized care plans, tailored to each resident's needs and goals
  • Collaborating with healthcare professionals, families, and other stakeholders to ensure comprehensive care
  • Managing medication, adhering to strict protocols and guidelines
  • Conducting regular risk assessments, identifying potential risks, and implementing strategies to mitigate them

**Requirements:

  • NVQ Level 3 in health and social care or equivalent experience
  • Strong organizational and leadership skills, with the ability to motivate and inspire others
  • Excellent communication and interpersonal skills, building relationships with residents, families, and colleagues
  • A good understanding of care planning processes, with the ability to develop and implement individualized plans
