Care and Support Coordinator Job Description:

We are seeking a skilled Care and Support Coordinator to join our team at National Care Group in Kidderminster. As a vital member of our support staff, you will play a critical role in coordinating care and support services for individuals with complex needs.

Main Responsibilities:

  1. Develop and implement individualized care plans to meet the unique needs of each individual
  2. Collaborate with colleagues, family members, and healthcare professionals to ensure comprehensive care delivery
  3. Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  4. Communicate effectively with stakeholders to ensure seamless care transitions

What We Offer:

  • An estimated salary of £28,000 - £35,000 per annum, based on experience and qualifications
