Community Mental Health Team Lead Role

This role offers an exciting opportunity for a highly skilled and experienced mental health care professional to join our Colchester Specialist Community Mental Health Team as a team lead.

The successful candidate will be responsible for leading a team of care coordinators, assessing new referrals, coordinating care needs, and planning and implementing evidence-based care plans for a designated caseload of service users and their carers.

A key aspect of this role is providing leadership and supervision to the team, as well as working effectively as part of a multidisciplinary team to provide high-quality care and support to service users and their carers.

A salary of £45,000 - £55,000 per annum is offered, dependent on experience.

We are looking for an individual with excellent leadership and communication skills, who is able to work effectively as part of a team and provide high-quality care and support to service users and their carers.

About the Job:

  • Leading a team of care coordinators
  • Assessing new referrals to the team and identifying care needs
  • Coordinating care and assessment under the Care Programme Approach (CPA)
  • Creating and updating person-centered care plans
  • Facilitating referrals to crisis teams or admission to hospital
  • Administering depot injections in a community setting

Requirements:

  • Registration as a Mental Health Nurse (RMN) Part 3
  • Good written and spoken English
  • Evidence of appropriate Continuing Professional Development (CPD) post-qualification
