Position Overview

This role presents a unique opportunity to enhance the transition for individuals dealing with substance use challenges from healthcare facilities to their communities. The position requires close collaboration with Substance Use Liaison Teams to evaluate and support service users, provide guidance and information, and manage a specific caseload of clients upon their release from healthcare settings. You will be part of a dedicated and welcoming team at Turning Point, employing a multidisciplinary approach supported by clinical and psychosocial intervention staff, peer mentors, and a robust network of partner organizations.

As a Community Support Liaison Specialist, we offer a competitive starting salary of £27,300, with annual salary progression reaching up to £31,937.

Key Responsibilities

To contribute to the implementation of Turning Point's Substance Use strategy by planning and delivering high-quality, innovative care that aligns with our person-centered values and our ambitious goals for the recovery of those we support.

Main Duties:

Provide in-reach services to emergency departments, medical wards, and gastrointestinal wards. Collaborate in the creation of aftercare plans alongside Hospital Substance Use Liaison Nurses prior to client discharge. Aid in the development of seamless care pathways for ongoing detoxification processes initiated in healthcare settings that are suitable for continuation in the community. Conduct assessments, facilitate early engagement, offer advice, and implement brief interventions for individuals identified as needing substance use support by Hospital Liaison Nurses, while developing community care packages. Engage with families and significant others of clients identified as needing support to maximize social recovery resources and involve them in recovery plans where feasible. Adapt to various settings to formulate care and support plans that cater to the needs of the service and clients in line with the recovery agenda.

Ideal Candidate Profile

You will possess an understanding of substance and alcohol use, along with the challenges and opportunities faced by individuals on their recovery journey. We also recognize that candidates from diverse backgrounds, such as mental health, housing, or other health and social care sectors, may bring valuable transferable skills. Flexibility, the ability to work dynamically in both individual and group settings, and knowledge of relapse and recovery barriers are essential, but we can assist in developing these skills as needed. Excellent communication skills and alignment with our organizational values are highly sought after. We appreciate individuals with lived experience, as well as those who are passionate about this field; our Trainee Recovery Worker program allows individuals without all relevant experience to enter the organization and progress to other roles.
